POLICIES

1– In the event that your insurance plan requires you to have a referral:

  • It is up to the physician to decide whether you need a referral or not. If you have an HMO/managed care plan, most insurance companies require us to treat you up to a reasonable extent in our reach. 
  • Kindly give us 5-7 or more business days to prepare your referral. 
  • If your insurance company delays/denies the referral, we will not be responsible for any inconvenience/expenses. 
  • We are unable to issue backdated referrals
  • Please pick up your referral from our office a few days before your visit with a specialist 

2– Referrals will be processed the same day only if it’s an emergency and we are sending you directly to see a specialist, please note: it is not an emergency if you are at the specialist’s office without a referral.

3– It is your responsibility to be fully familiar with your insurance coverage and benefits. Due to varying insurance company requirements, we cannot be responsible for referring you to a facility not covered by your insurance company (For Example: for blood work, x-ray, specialist or another paramedical specialty). Prior to visiting any facility or doctor for further workup or a treatment, please confirm with your insurance or related facility.

4– If you have not been seen by our doctor in more than 3 months or have not been seen recently for the condition you are seeking to see specialists for, you will need to be seen first in order for us to process your referral.

Missed / Cancelled Appointments

As a courtesy, we make reminder phone calls the day before an appointment. However, it is the patient’s responsibility to remember appointment and be on time. We charge $30.00 for any broken appointment without 24 hours’notice. 

We try to make every attempt to maintain a timely schedule. This is not always possible. We believe that it is of the utmost importance to give our full attention to each patient and family and assure that all questions and concerns are addressed. This may sometimes cause a slight delay. In trying to maintain our schedule, some difficulty may arise when a patient arrives late. We understand that the reasons may be numerous. Again, we ask for the courtesy of calling our office, notifying us that there is a delay and providing an approximate time of arrival. If you will be more than 30 minutes late for your scheduled appointment, your practitioner will be informed, and she will determine whetherit is better to have the appointment rescheduled for later the same day or for another day.  


Forms: School / Work/ Disability Etc

Requests for completion of any forms, reports or other paperwork may require up to 7 or more business days and a fee of $ 15/per form, paid in advance, proportional to the amount of the preparation involved. Kindly realize that filling out forms is never an emergency. We request you not to hold on to forms and rush to have them completed at the last minute. Schools and other educational/recreational facilities do give forms sufficiently in advance so that doctors have adequate time to complete them. Certain forms may require a visit to our office

     NO FORMS WILL BE FILLED OUT DURING THE APPOINTMENT 

Medication Refills:  

Please make sure you have a sufficient supply of your maintenance medication, there will be no refills after hours/ on weekends or holidays.

In some cases, you would need to be seen before we refill your medications. These visits are made to ensure that the medication is working properly, monitor for adverse reactions, maintain effective dosing. These appointments are generally every 3 to 6 months depending on your condition.  
For all controlled substances (pain medications, anxiety/ ADHD and antibiotics): these types of medications require an office visit every time they are prescribed.

Medical Records Release: 

As a courtesy to our patients who need to change their physician, we will provide the medical record summary for one time ONLY. We need a written request and a minimum of seven to ten working days or more to review the charts and prepare the medical record summary. These records will be mailed directly to another physician’s office. Release forms must be signed before we can send records.
